In accordance with IRS regulations, Tomo Drug Testing will not automatically mail Form 1095-C to employees or former employees.

Form 1095-C provides information about the health coverage offered to you during the applicable tax year.

If you would like to receive a copy of your Form 1095-C, you may request one at no charge by contacting:
